a day). Fax orders must include purchase order number or credit card number
and expiration date to be processed. NOTE: If you have placed an order
by phone or fax, PLEASE DO NOT MAIL IN THE SAME ORDER. This may cause duplicate
orders.
IN PERSON: If you are in the Oklahoma
City area, stop by our showroom located 1.5 miles east of
I-35 at 3405 S.E. 29th and an account
manager will assist you.

Include the following information
when ordering:
Catalog product number(s).
For all orders which require typesetting
or engraving, complete the attached form and mail or fax with your order
form. This step in procedure is critical to producing your order correctly
and on time.
Shipping and billing information
along with your customer number.
Date products are needed.
Name and telephone number of person
placing order.
For trophies or plaques, indicate
the figurine desired.
Any artwork desired on award.
You can supply MTM with camera-ready artwork (black and white only, no
fades, blends, percentages or halftones), or choose from a wide variety
of stock art. Laser prints, faxes or copies are not camera-ready, and will
require a minimum art charge of $45.00 to ready the artwork for production.The
type of file required for the method of production will vary from order
to order. For maximum results, contact an account manager and ask to speak
with one of our graphic artists.

Artwork will be accepted in the
following formats.

With every art file, include a text
file describing fonts used in the document, application used, and other
notes you wish to communicate. Compress each file with Stuffit (Mac) or
Winzip (Windows). Customer provided artwork may not be considered camera
ready. The Art Department will decide if the art is camera ready. Art
charges will not necessarily be waived. If the artwork is determined to
be camera ready, the art department will notify the sales manager to waive
the art charges. If you will be sending an e-mail, include the Account
Managers name in the subject field. This and more detailed information
about preparing artwork or typeset files can be found on our website. Please
review this information before preparing and sending your files.
You may also supply the
artwork on disk, in the same formats listed above. Disk Drives: 3 1/2
disk, Zip, Jazz, or CD rom. All supplied disks are copied by MTM and the
original is returned. Any questions regarding disk requirements should
be addressed through your account manager to the graphic arts department.
Please provide a hard copy of
the art (preferably camera-ready) and a layout detailing PMS#s and typefaces.
Hand-drawn illustrations must
be black and white line art.
MTM can produce your logo in four
color process. This process allows a multitude of colors in fewer screens.
You must talk to an account manager and a graphic artist to determine the
requirements of the job and quote.
If a file has an imported or placed
image, they MUST be supplied on the disk.
Please provide all fonts on disk,
including the screen font and printer font, or you can convert the fonts
to curves/paths. This, however, will limit us from making any adjustments
or changes. Any specifications not met may require art clean up or rework
charges (we will call to inform you of any rework charges) and could delay
production.

Copy and trademarks represented
in this catalog
are for purposes of demonstrating
product only and do not constitute endorsement or advertisement of any

product line by a company or other organization.
By submitting artwork, designs,
or other copyrighted or trademarked material concepts for reproduction
in any form, you are representing that the concept provided is either your
original concept or that you have permission from the original creator
to reproduce it and that it is free from potential copyright, patent, or
trademark claims. By submitting a concept you are agreeing to indemnify
Midwest Trophy from any and all such claims that may arise from such reproduction.
Cancellations
Orders cancelled during production
will be
charged for the work completed at
the time of cancellation.
Voice Mail
To check the status of an order,
call our customer service department at (405) 670-4545 ext. 472. MTM has
a voice mail system for your convenience. If you reach your account managers
extension and find it busy or your account manager is unavailable, please
leave a detailed message including your phone number and any specific information
desired. Your account manager will return your call.
Drop Ship Service
MTM can economically provide drop
shipment service and/or custom packaging of products other than our standard
pack. Call or write with your specific details and a price will be quoted.
Warranty
All products sold by MTM are guaranteed
to be of good quality materials and workmanship. If our quality inspectors
overlooked a factory defect, write or call your account manager within
ten days after delivery and we will issue a Returned Goods Authorization
(RGA) number for return. All shortages in shipment must be reported within
10 days of delivery to receive credit replacement.
Returns
A Return Authorization (RA) number
must be issued
for all returns. Merchandise without
a RA will not be accepted. Custom orders may not be returned unless a factory
defect exists. A 20% restocking fee applys to all returns
Payment Information
Credit Terms
All orders must be either prepaid
cash, check, money order, credit card, or C.O.D. MTM will extend credit
however, a credit application and billing agreement must be filled out
and approved. Allow 10 days for credit approval. Terms are net 10 days.
Finance charges will be assessed at 1.5% per month after an account is
30 days past due. If you plan to charge to VISA, MasterCard, American Express,
or Discover, please have your card number and expiration date available
when placing your order.
Method of Payment
PURCHASE ORDER: If you are not prepaying
by check or charging your order on American Express, VISA, Mastercard or
Discover, you must submit a valid purchase order number from your school
or organization. Please place customer account number on all correspondence.
CREDIT CARD: To place an order using
a credit card we require the type of card, card number, name as it appears
on the card, expiration date and home phone number. Please place customer
account number on all correspondence.
CHECK: Make payable to Midwest Trophy
and be sure to include shipping and handling. (Oklahoma, Nebraska,
and Texas, add applicable state sales tax.) Please place customer account
number on all correspondence.
C.O.D.: There is an additional charge
for C.O.D. shipments. Payment is required upon delivery. Be sure to get
estimated shipping costs from your account manager. Please place customer
account number on all correspondence.
MONEY ORDER: Make payable to Midwest
Trophy. Please place customer account number on all correspondence.
Shipping Information
Merchandise will be shipped
via ground where
weight limits permit. Unless otherwise
specified, shipping will be F.O.B. factory. MTM is not responsible
for delays or damage in transit.
Prepaid Shipments
Priority shipping is available;
however, customer is
responsible for all shipping charges.
Orders sent open account will
be billed shipping charges, which includes prepaid shipping insurance.
Prepaid Orders
Prepaid orders must follow
the shipping schedule
on the attached order form. Oversized
or extra heavy parcels will require additional charges and the customer
will be billed any extra shipping costs.
Method of Shipment
F.O.B.: Factory
GROUND: This is the normal method
of shipment unless otherwise specified. (Please give street address
for delivery not P.O. Box) Ask your account manager for average delivery
time to your area.
2nd DAY SERVICE: This method assures
the merchandise to be delivered by the close of business on the 2nd day
(Mon-Fri) to most destinations in the continental U.S. Additional
shipping costs apply.
NEXT DAY DELIVERY: This method assures
the merchandise to be delivered the next day after shipment to most destinations
in the continental U.S. Additional shipping costs apply.
